- Utilize advanced clinical expertise to deliver high-quality, evidence-based patient care.
- Perform routine assessments, monitor vital signs, and evaluate complex patient conditions.
- Manage patient access to healthcare and connect individuals with community resources.
- Conduct medication reviews to identify potential side effects and drug interactions.
- Develop and administer mental health assessments to screen for depression and evaluate mood, behavior, and cognitive function.
- Accurately document patient medical history using electronic health records.
- Schedule: 6-week blocks following 3 weeks of training/orientation; shifts between 7-10 hours, including weekdays and every other weekend; includes major holidays.
- Remote/on-site/hybrid: Onsite as needed.
- Shift/hours, compliance/submission requirements, environment notes included.
- Active and unrestricted license as a Licensed Practical or Vocational Nurse in the United States or District of Columbia.
- High School Diploma or GED verified.
- Experience with phlebotomy, vaccine administration, and point-of-care testing.
- Minimum of one-year related healthcare work experience.
- Proficiency with electronic medical record (EMR) systems; Epic experience preferred.
