- Provide primary and family evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for patients.
- Perform administrative duties including opening/closing the clinic, inventory management, and patient follow-up.
- Handle patient appointments and perform routine examinations to ensure positive health outcomes.
- Encourage patients to participate in their own health care through services such as health screenings and chronic condition management.
- Coordinate with other healthcare professionals to maintain a comprehensive plan of care.
- Work in collaboration with a dedicated clinical care team to drive better outcomes for patients.
- Work hours are Monday to Friday, 8:30 AM to 4:00 PM; orientation is 4 weeks at 40 hours per week.
- This is an in-person, patient-facing role.
- DEA registration may be required during onboarding.
- Requires working three out of every six weekends with possible holiday rotations.
- Master's Degree from a Family Nurse Practitioner program with current National Board Certification.
- State of Employment license to practice in the Advanced Practice Nurse role.
- Minimum of 1 year of recent primary care experience as a Nurse Practitioner.
