 Hybrid Case Worker Hours

• 8a – 4:30pm (30 min lunch) Monday to Friday
• Partial telework is available (hybrid/work-from-home days)

 Hybrid Case Worker Compensation

• The pay for this position is between $25+ an hour, depending on experience
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

 Hybrid Case Worker Responsibilities

• Reviews and recommends modifications to program manuals and materials based on policy revisions. Develop and update forms and files.
• Coordinates the collection of Data to prepare reports.
• May be Involved in incident reviews, closing incidents, reports and investigations.
• Review previously initiated protective services cases and perform investigative activities to complete the investigation.
• Identify any unmet needs or potential risks to the older adult.
• If any unmet needs are identified or any potential risks identified, refer the case back to the AAA for services (preparation and implementation of a PS service plan).
• If no unmet needs or potential risks are identified, close the case record in accordance with the PS database manual.
• Document all case-related actions and results (i.e. interviews with collateral contacts, review of medical or financial records, obtainment of a Power of Attorney document, etc.) and enter all required information into the PA Department of Aging’s standardized documentation system.

 Hybrid Case Worker Requirements

• Bachelor Degree in human services, social work, psychology, sociology, or similar
• 2 years either in research or providing social services required
• 1 year of direct aging casework preferred
