Job Description
The Addiction Counselor will provide counseling and support to individuals struggling with addiction, helping them navigate the path to recovery.
Discharge Planner / Corrections Social Worker / Corrections Counselor / Drug Recovery and Resettlement
Looking for a Human Services or Mental Health Professional to work directly with prisoners that have a history of drug abuse, assist with discharge planning, MAT therapy and appointments, coordinating community resources are in place for reentrants on the date of their release.
TRAINING:
- On-site training provided during shift times on location
AND
- DOC training will be required with in the first 6 months of starting the role. This will be 3 weeks of required training to be completed in Elizabethtown, PA.
- Travel will be reimbursed
- Overnight stay will be provided to stay during the week, Saturday and Sunday will be at home.
- Report time on Monday 11am, release time on Friday 12pm to give time for driving. (will be paid for full days on these days)
Hours: 8a – 4pm (30 min lunch), Monday – Friday
Location: Karthaus, PA
Pay: $27.00-28.00
Job Duties:
- Implement and provide social work, counseling, and case management services to individuals to enhance their social functioning and to help them attain a more satisfactory social, economic, or physical adjustment.
- Assist with referrals in the community, including setting up and scheduling appointments for medication and treatment.
- Ensure needs, benefits, and services are obtained in accordance with assigned individuals’ treatment plans.
- Track and update spreadsheet/database on everyone released from the facility
- Meet with reentrants with positive urine drug screens to assess and refer to treatment.
- Complete assessments as required (Initial, 6 months, discharge).
- Communicate all treatment planning to Center Staff, SCI Staff, and/or Parole staff.
Requirements:
- Bachelor’s in Social Worker or Human Service or Similar field required.
- Knowledge and experience in Case Management, Discharge Planning and Community Resources.
- Knowledge of counseling techniques and interventions used to recognize or improve psychosocial, economic, and physiological situations of individuals and families.
- Knowledge of commonly occurring physical and mental illnesses and their social, economic, and psychological implications.
