Case Manager – Populance Job Description:

The Case Manager is a vital member of the ambulatory care team, responsible for short-term patient enrollment into case management services and applying appropriate billing codes. This role emphasizes telephonic outreach, patient advocacy, and coordination of care to support health outcomes across the continuum. The position uses an electronic health record system (training provided) and requires strong communication, organizational, and decision-making skills.

Case Manager – Populance Schedule & Location:

• Monday–Friday, 8:00 AM to 5:00 PM

• 13-week contract

• Local candidates required unless previously completed Red Cross billing training

• Remote eligible for those already Red Cross trained

Case Manager – Populance Salary/Wage:

• 35.00/hr - 40.00/hr 

Case Manager – Populance Duties:

• Enroll patients into ambulatory case management services

• Allocate appropriate billing codes to services

• Conduct targeted telephonic outreach to meet performance metrics

• Perform motivational interviewing, care planning, and goal setting

• Coordinate care and connect patients with community resources

• Use Epic Compass Rose for documentation (training provided)

• Promote patient safety, quality of care, and cost-effective outcomes

• Evaluate and plan care based on social determinants of health

Case Manager – Populance Required Qualifications:

• 3–5 years of telephonic case management experience (inpatient experience not accepted)

• Must be a Registered Nurse (RN) or Licensed Master Social Worker (LMSW) with a valid, unrestricted State of Michigan license

• Must complete Red Cross billing/coding training within 6 months (unless already certified)

• Familiarity with:

  • Motivational interviewing

  • Goal setting and care planning

  • Care coordination

  • SDoH assessment and resource referral

• Experience with:

  • Patient Centered Medical Home model

  • Provider Delivered Care Management billing

  • Case management billing codes

  • MICMT training (Team-Based Care or Patient Engagement Foundations)

  • Epic or similar electronic documentation system

  • Working in a fast-paced environment under performance metrics

Case Manager – Populance Physical Requirements:

Case Manager – Populance Education:

• Bachelor’s degree in Nursing or a related field OR Master’s degree in Social Work

• Case Management Certification (CCM or ACM) required within 3 years of hire
