Behavioral Health Case Manager Hours

• 8a – 4pm (30 min lunch)
• 3-week training in Elizabethtown

Behavioral Health Case Manager Compensation

• The pay for this position is $26.00-28.00 an hour.
• Benefits are available to full-time employees after 90 days of employment.
• A 401(k) with a company match is available for full-time employees.

Behavioral Health Case Manager Responsibilities

• Assist with MAT referrals in the community, including setting up and scheduling appointments for medication and treatment.
• Ensure needs, benefits, and services are obtained in accordance with assigned individuals’ treatment plans.
• Track and update spreadsheet/database on everyone released from an SCI on a MAT.
• Shall attend weekly Vivitrol Conference Call, and other meetings as needed.
• Document all contacts in appropriate systems (Captor, Sapphire etc.).
• Attend all required trainings.
• Meet with reentrants with positive urine drug screens to assess and refer to treatment.
• Complete the GPRA as required (Initial, 6 months, discharge).
• Meet with Reentrants in the SCI, BCC, Parole Offices, and/or Treatment providers as necessary.
• Communicate all treatment planning to Center Staff, SCI Staff, and/or Parole staff.
• Subscribe to the MAT/AOD Treatment framework and policies and procedures of the DOC.
• Implement and provide social work, counseling, and case management services to individuals to enhance their social functioning and to help them attain a more satisfactory social, economic, or physical adjustment.
• Perform in accordance with applicable rules, regulations, policies, and procedures under the general supervision of an administrative supervisor.
• Assess offenders’ psychosocial strengths, weaknesses, and social work service’s needs, and eligibility for services.
• Counsel individuals utilizing various treatment modalities.
• Discuss assigned individuals’ psychosocial assessments and family history in multidisciplinary treatment team meetings.

Behavioral Health Case Manager Requirements

• Possess bachelor’s degree in social work, psychology, sociology, human services or similar
• 1 + years of case management
• Must be able to work in correctional environment.
