Bilingual Case Management Coordinator Compensation
- The pay for this position is $34.13 per hour.
- Benefits are available to full-time employees after 90 days of employment.
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates.
Bilingual Case Management Coordinator Highlights
This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs.
The required availability for this position is Monday – Friday, 8:00 AM – 5:00 PM. Candidates must be able to travel approximately 50–75% of the time within Miami-Dade County.
Bilingual Case Management Coordinator Responsibilities

Conduct telephonic and face-to-face visits with members in their homes, Assisted Living Facilities, and Skilled Nursing Facilities.

Assess, plan, implement, and coordinate all case management activities to evaluate the member’s medical and supportive needs.

Coordinate Prior Authorizations, PCP and provider communication, condition management, medication reviews, and community resources.

Develop a proactive course of action to address member issues and enhance overall wellness.

Document all case management activities in accordance with company policies and regulatory guidelines.

Provide coaching, education, and support to empower members in healthcare decision-making and lifestyle choices.
Bilingual Case Management Coordinator Requirements

Bachelor’s degree in Social Work or a related field required.

Must reside in Miami-Dade County.

Fluent in Spanish and English (speaking, reading, and writing).

Ability to multitask, prioritize, and adapt to a fast-paced environment.

Effective verbal and written communication skills.

Case management experience required; Long Term Care experience preferred.

Proficiency in Microsoft Office, including Excel and Teams.
Bilingual Case Management Coordinator Preferred Qualifications

Experience with community-based care coordination.

Experience working with medically complex populations.
