Location: Hialeah, FL. MUST live in one of these zip codes: 33010, 33002, 33011, 33012, 33013, 33014, 33015, 33016, 33017, 33018
- Travel:50% to 75% local travel for member visits (mileage reimbursed)
Pay: $34.13/hr.
Schedule: Mon-Fri, 8:00am-5:00pm. 4-6 weeks remote training (Microsoft Teams)
Overview
We are hiring a Healthcare Consultant/Case Management Coordinator to support Medicaid Long Term Care program members. This is a NON-NURSING work-from-home role with significant in-person member visits in the Hialeah area (homes, assisted living facilities, and nursing homes).
Key Responsibilities
- Coordinate case management activities for Medicaid Long Term Care/Comprehensive Program members
- Perform telephonic and face-to-face assessments and ongoing care coordination
- Develop, implement, and monitor care plans to support member wellness outcomes
- Collaborate with PCPs and provider teams to coordinate services and supports
- Support prior authorizations, medication reviews, condition management, and community resource linkage
- Identify and escalate quality of care concerns through established channels
- Use motivational interviewing & member engagement strategies to drive adherence & healthy behavior change
- Maintain documentation & compliance with all guidelines
Job Requirements
Required Skills and Qualifications
- Bachelor’s degree required (social work degree or related field)
- Bilingual Spanish/English required (read, speak, and write fluently)
- 1+ year of relevant case management experience
- Long term care & Managed care experience
- Case management and discharge planning experience
- Case management certificate (preferred)
