Physical Therapist Position Summary:
We are seeking a dedicated and skilled Physical Therapist to join our Home Health team. This role involves providing physical therapy to patients of all age groups in their homes, working closely with an interdisciplinary team to develop and implement patient care plans aimed at restoring or maintaining patient well-being. The Physical Therapist will be responsible for conducting clinical assessments, providing therapeutic treatments, and educating patients and families on care techniques.
Physical Therapist Key Responsibilities:
- Perform clinical assessments to determine patient needs and the appropriateness of therapy.
- Develop, document, and continuously review patient Plans of Care (POC), ensuring it is updated based on ongoing assessments and feedback.
- Provide therapeutic treatment for neuromuscular, neurological, and orthopedic conditions in line with medical referrals.
- Instruct patients and families on transfer techniques, safe use of mobility aids (e.g., walkers, wheelchairs), and other therapeutic equipment.
- Educate patients and their families on therapy procedures, tailored to the patient’s age, condition, and cultural background.
- Apply infection control and universal precautions in all patient care activities.
- Coordinate with other healthcare team members to ensure quality care.
- Perform other duties as assigned or requested.
- Bachelor's degree in Physical Therapy from an American Physical Therapy Association (APTA) accredited program.
- Current state license as a Physical Therapist.
- Proof of current CPR certification.
- Valid driver’s license with reliable transportation and insurance.
- At least 1-2 years of experience in an acute care or rehabilitation setting required.
- Previous home health experience preferred.
- OASIS experience is highly desired.
- Strong communication and interpersonal skills.
- Knowledge of durable medical equipment.
- Ability to work independently and as part of a team.
- Ability to lift, bend, pull, and push, with prolonged periods of standing and walking.
- Commitment to professional growth and competency in the field.
- Ability to work flexible hours and travel locally.
- No on-call or weekend shifts.
- Guaranteed hours are negotiable.
- 28 units per week.
- Flexibility to float to the Hope, AR branch if needed.
Physical Therapist Pay Rate: $50 to $70 per hour, depending on experience.
