Care Management Associate Compensation

• The pay for this position is $18.34 per hour
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Care Management Associate Highlights

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• The required availability for this position is Monday-Friday, 8am-5pm
• This position is almost entirely WFH/Remote!
• Employees will only be required to go onsite for quarterly meetings (once every 3 months)

Care Management Associate Responsibilities

• Outreaching members to complete Health Risk Assessment (HRA).
• Entering mailed in HRAs and following up on any care coordination needs identified.
• Outreaching members within 3 days of hospital discharge to complete a Post Discharge Questionnaire (PDQ) to prevent hospital readmissions.
• Reviewing Low Risk Care Plan goals with members and referring to PCP and/or CM for any unmet goals.
• Obtaining a Member Centered Goal as per new CMS guideline.

Care Management Associate Requirements

• 1+ years’ experience in healthcare in a customer service or patient coordinating role
• Strong understanding of medical terminology and/or care management
• Computer literacy to navigate through internal/external computer systems
• High School Diploma or GED
• Attendance is mandatory for the first 90 days

Care Management Associate Preferred Qualifications

• Any care management experience is a plus
