A-Line Staffing is now hiring a Care Manager II (RN) in Dallas TX United States. The Care Manager II (RN) would be working for a Fortune 500 company and has career growth potential.

Care Manager II (RN) Compensation

• Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Care Manager II (RN) Highlights

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• NO WEEKEND SHIFTS! Monday through Friday only!
• Remote -Field- will be doing in home assessments for members 4 days per week
• Dallas TX only
• 1 day per week will be working on documentation in remote home office.
• Candidate will need to be able to drive self in personal vehicle to the member homes. Travel will include the following counties: Dallas, Collin, Rockwall, Hunt, Navarro, Kaufmann, Ellis.
• Regular Pay: $45/hr

Shift: M-F 8-5pm (one hour lunch).

Care Manager II (RN) Responsibilities

• Potential for face-to-face assessments in member home within the Dallas SDA- Dallas, Collin, Rockwall, Hunt, Ellis, Navarro, Kaufman counties as scheduled
• Complete all documentation in the electronic health record; referrals for items/services, coordination of care
• Following up on members in the upgrade process to implement services
• Making outbound phone calls to members and providers to coordinate services

Care Manager II (RN) Requirements

• Licensed TX Rn – No restrictions
• Some from home heath, some hospital, some care management
• Computer literacy, Excel, Outlook, Word
• Valid Drivers License
• Remote experience preferred
• Prior home health care experience
• Bilingual- Helpful but not necessary
• Current RUG certified is preferred
