A-Line Staffing is now hiring RN Case Manager in the Waco, TX 76704 area. The RN Case Manager would be working for a Major Health Plan and has career growth potential. This would be full time / 40 hours per week.

RN Case Manager Compensation:
  • The pay for this position is $35 - $45 hourly (plus mileage reimbursement for fieldwork)
  • Benefits are available to full-time employees after 90 days of employment
  • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

RN Case Manager Highlights:
  • The required availability for this position is as follows: Mon-Fri 8am-5pm
  • Hybrid role based in Waco and the surrounding counties would be Navarro, McLennan, Hill, Ellis, Limestone, TX - Member facing/Field worker expectation is 50/50 hybrid.  At times one may be asked to travel 2-3 hrs away

RN Case Manager Responsibilities:
  • Perform duties related to the day-to-day operations of the Service Management functions for Foster Care members to achieve the highest possible levels of functioning and wellness by ensuring consistent, timely access to medically necessary physical health services. Monitor quality of care, assist with discharge planning.
  • Performance expectations/metrics:

    • Work with Foster Care Members, Medical Consenters, Department of Family and Protective Services (DFPS) staff and providers to meet the healthcare needs of each foster care member’s condition

    • Conduct health assessments IN-PERSON and identify foster care members who may benefit from service management, conduct an assessment and develop, implement and monitor a healthcare service plan.

    • Provide information to DFPS staff to facilitate development of the DFPS case plan.

    • Update database regularly with most recent Service Management Plan of Care.

    • Review information submitted by providers into the database to ensure material is appropriate and consistent with guidelines for data inclusion.

    • Encourage health providers to use evidence-based practices and confirm that health providers and PCP’s are sharing information as required.

    • Comply with established referral, pre-certification and authorization policies, procedures and processes by related medical management staff.

    • Maintain compliance with federal and state regulations and contractual agreements.

    • Coordinate service management functions with other departmental functions.

    • Monitor data to address trends or potential quality improvement opportunities and maintain HIPPA compliance.

RN Case Manager Requirements:
  • 3+ years' experience working with children and adolescents with special needs health conditions within CPS, DFPS, child welfare systems, or local and state mental health authorities
  • Associates in Nursing or Bachelor of Science in Nursing
  • Must have active RN license with Texas nursing board.
  • Must have active driver’s license and reliable transportation.
  • Case Manager or Service Coordinator background
  • Comfortable with Microsoft Suite (Excel, Word, Teams, Outlook) and Zoom.

RN Case Manager Preferred Qualifications:
  • Pediatric Nursing and/or Emergency Department/ICU
  • Hospital experience is valuable
  • Case manager with a health plan
