Full time Social Worker / Care Manager openings in the Dallas, Texas area with a major health care company!!! Starting ASAP!

**Position is hybrid with 3 days of travel and 2 days of remote work per week, travel will be in the Dallas Metro area**

Social Worker / Care Manager PAY: $25.00-$28.00 per hour

Social Worker / Care Manager SCHEDULE: M-F 8am-5pm Central Time

Social Worker / Care Manager Job Description:

• 3 days of visits and 2 days of documentation weekly
• Will have a caseload and work assigned tasks
• Complete H2060 assessments with members in managed care services
• Make follow up calls

Describe the performance expectations/metrics for this individual and their team:

• Complete documentation within 3-day turnaround time
• Stay organized and ensure work they do not fall behind
• Maintain a 90% or high in Audit scores

Social Worker / Care Manager Required Skills:

• Ability to travel 3 days per week (will be reimbursed for mileage)
• Bachelor’s degree in Social Work, Health Services, Behavioral Science or related field, or equivalent experience.
• Will also consider LVN with Case management experience and no degree
• Strong documentation skills
• Excellent computer skills
• High School Diploma or GED

Social Worker / Care Manager Preferred Skills:

• LVN OR Bachelor's Degree required in Social Work or related field
• Service Coordination, Case Management or Nursing experience preferred.
• Bilingual candidates highly preferred
• Background in Managed Care
• Familiarity and experience working with Medicare or Medicaid plans
• Degree preferred
