A-Line Staffing is now hiring RN Case Manager in the Dallas, TX area. The RN Case Manager would be working for a Major Health Plan and has career growth potential. This would be full time / 40 hours per week.

RN Case Manager Compensation:

• The pay for this position is $38 - $46 hourly (plus mileage reimbursement for fieldwork)
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

RN Case Manager Highlights:

• The required availability for this position is as follows: Mon-Fri 8-5 with a 1-hr lunch
• Hybrid role/1 day remote based in the Dallas, TX area - Member facing/Field worker willing to travel doing in home assessments for members 4 days per week.

RN Case Manager Responsibilities:

• Care Manager would be responsible to complete in home assessments 4 days per week with our members in their home, 1 day remote.
• Potential for face-to-face assessments in member home within the Dallas SDA- Dallas, Collin, Rockwall, Hunt, Ellis, Navarro, Kaufman counties as scheduled
• Complete all documentation in the electronic health record; referrals for items/services, coordination of care
• Following up on members in the upgrade process to implement services
• Making outbound phone calls to members and providers to coordinate services

Performance expectations/metrics:

o   Need to be able to use Excel, use spreadsheets to determine productivity

o   Have to be able to use telephone and Microsoft Office, Outlook is a plus because they are coordinating member services

o   Candidate needs wifi at their home during WFH.

o   Will be flexible assignments will be based on business needs

RN Case Manager Requirements:

• Must have active RN license with Texas nursing board.
• Must have active driver’s license and reliable transportation.
• Case Manager or Service Coordinator background
• Some home heath, some hospital, or some care management is acceptable
• Comfortable with Microsoft Suite (Outlook, Teams).

RN Case Manager Preferred Qualifications:

• Prior home health care experience
• Bilingual- Helpful but not necessary
• Current RUG certified is preferred
