Case Management Coordinator – Field-Based (Bilingual Spanish Required)

Location: Candidate MUST reside in Hardee or Highlands County, FL 
Pay Rate: $34.13/hour
Schedule: Monday – Friday | 8:00 AM – 5:00 PM
Remote: Fully remote (field-based, no on-site requirement)

Job Description:

A-Line Staffing is currently seeking a Bilingual Spanish Case Management Coordinator for a fully remote, field-based opportunity in Hardee or Highlands Counties. This role supports Medicaid Long Term Care/Comprehensive Program enrollees by providing proactive case management that promotes wellness, improves health outcomes, and connects members to needed services and resources.

Key Responsibilities:

• Conduct telephonic and in-person assessments to determine member needs

• Develop and implement care plans based on evaluation outcomes

• Coordinate with PCPs, specialists, and community providers

• Manage caseloads including both supportive and medically complex members

• Monitor progress, evaluate care effectiveness, and adjust plans as needed

• Use motivational interviewing techniques to engage members

• Provide education, coaching, and support for lifestyle and treatment adherence

• Document all member interactions and care coordination activities

• Escalate quality of care concerns as necessary

• Ensure compliance with all regulatory and policy guidelines

Qualifications:

• Bilingual in Spanish and English (required)

• 1+ year of case management experience (required)

• Bachelor’s degree required (Social Work or related field preferred)

• Strong Microsoft Office and Excel proficiency

• Experience with long-term care, Medicaid populations, or managed care (preferred)

• Case Management Certification (preferred)
