Case Manager – Populance Highlights
- The pay for this position is $37.52 - $38.54.
- Monday – Friday, 8:00 AM – 5:00 PM
- Hybrid/Remote (local Michigan candidates only unless billing/coding training is already completed)
- This position is a 13-week contract assignment
Responsibilities
- Provide short-term case management support in an ambulatory setting by enrolling patients into case management services and assigning billing codes.
- Perform assessment, planning, care coordination, facilitation, and evaluation for patients across the continuum of care.
- Conduct targeted telephonic outreach and support patients through complex care needs and resource navigation.
- Utilize Epic’s Compass Rose system to accurately document care plans and interventions (training provided).
- Engage in motivational interviewing, goal setting, care planning, and social determinants of health (SDoH) assessments.
- Collaborate with internal and external care teams to ensure high-quality, patient-centered outcomes.
- Advocate for patients and families using evidence-based practices and appropriate community resources.
Requirements
- Bachelor’s degree in nursing, social work, counseling, or related field OR Master’s in Social Work (MSW) required.
- Active, unrestricted Registered Nurse (RN) or Licensed Master Social Worker (LMSW) license in the state of Michigan.
- Minimum 3 years of clinical experience required.
- 3–5 years of telephonic case management experience (inpatient CM does not apply).
- Strong documentation and communication skills, with proficiency in Microsoft Office.
- Must be able to complete Red Cross billing/coding training within 6 months of hire (unless previously completed).
- Familiarity with motivational interviewing, goal setting, care planning, and care coordination.
- Knowledge of Patient Centered Medical Home (PCMH) and Provider Delivered Care Management (PDCM) billing preferred.
- Experience with Epic charting systems preferred.
- Ability to meet performance metrics in a fast-paced environment.
- Must possess qualities of empathy, flexibility, teamwork, and respect for diversity.
