Community Health Worker Hours
· M-F 8-5
Community Health Worker Compensation
- The pay for this position is $25.00 per hour
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Community Health Worker Responsibilities
· CHW works to promote healthcare equity and assist in addressing Social Determinants of Health for our Maryland Medicaid members.
· CHWs help connect our members with community resources (ex. Finding health care providers, arranging transportation to appointments, connecting with a food pantry), provide member education related to their health care and available benefits, and identify ways to improve their access to care.
· This role will be outreaching members to see how we can help meet their identified needs, and will be working in conjunction with our care management department.
· The majority of this role will be out in the field with the remaining time in the home office completing documentation and conducting telephonic outreach.
Community Health Worker Requirements
· High School Diploma or GED for minimum requirements
· Bilingual in Spanish is preferred
· Experience working with Medicaid populations in urban or rural environments
· Experience working in maternity and/or community health
