Job Description
A-Line Staffing is now hiring Certified Community Health Worker in the Orlando, FL area. The Certified Community Health Worker would be working for a Fortune 500 company and has career growth potential. This would be full time / 40 hours per week.
**Must reside in Orange, Hillsborough, Polk, or Osceola County of Florida - Position will be hybrid and will require 80% local travel in those areas conducting member-facing visits; mileage reimbursement is provided**
Certified Community Health Worker Compensation:
- The pay for this position is $26 hourly, plus mileage reimbursement
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Certified Community Health Worker Highlights:
- The required availability for this position are as follows: Monday - Friday 8am – 6pm
- Hybrid role: field work for member outreach, community engagement, care planning, and assessment completion. mileage reimbursement and computer equipment provided.
Certified Community Health Worker Responsibilities:

Efforts will be tailored round key deliverables for the pilot

Opportunities for program development, leveraging best practices and direct feedback on program experience

Outcome reporting (i.e. outreach efforts, successes, barriers, etc.)

Utilization of clinical documentation system
-
Member/Provider Outreach (telephonic + face to face)
-
Serving as the health plan liaison for new/existing community based organizations
-
Completion of health risk assessments/screenings
-
Addressing social barriers (i.e. closed loop referrals)

Performance expectations include, but not limited to:
-
Member and/or Provider Outreach compliance

Quality Case Review Audits
-
Defined Key Performance Metrics specific to the pilot (i.e. reduction in unable to reach population, reduction in ER visits, etc.)

Certified Community Health Worker Requirements:
- Community health worker CHW Certification
- Relevant case management, community engagement, care planning, assessment completion
- Valid driver's license and reliable transportation
- High-speed wired internet, and quiet/secure home work space
- Working Knowledge of Social Determinants of Health (SDOH) barriers
- Working Knowledge of geographic area(i.e. posted county/service area) and known Community Based Organizations
- Working knowledge of motivational interviewing and care plan development/goal setting.
- Requires at least an High School diploma or GED
- General understanding of Microsoft office suite (i.e. work, outlook, excel)
