- Ensure the client's plan of care is individualized, appropriate, executed, and reassessed as needed.
- Review evaluation packets, OASIS, and 485 forms for accuracy and follow up on documentation corrections.
- Process OASIS and 485 forms, verify start of care and episode dates in HCHB.
- Manage referral intake to ensure assessment visits are scheduled and completed timely.
- Lead weekly case conferences, review and approve notes, and address recertification or discharge decisions.
- Review and approve orders, ensuring they are correctly documented, updated, and routed for physician signature.
- Follow up on vital sign alerts, wound score deviations, and other clinical documentation and patient care needs.
- Schedule: Monday - Friday, 8:00 am - 5:00 pm, full-time, in-office position.
- Remote/On-site: In-office with clinical responsibilities; may perform patient visits and participate in on-call rotation.
- Compliance: Must adhere to federal, state, and local home health regulations.
- Graduate of an approved nursing school with an active RN license in the state of employment.
- Minimum of two years nursing experience with at least one year in home health.
- Valid driver's license and automobile liability insurance.
- Experience with Oasis, HomeCare HomeBase, and home health operations preferred.
- Benefits are available to full-time employees after 90 days of employment.
