Compensation:

  • The hourly pay for this position is $21.38/hr-$26.13/hr based on experience

Schedule:

  • M-F with possible Sat shifts 40 hours/week.
  • The hours of operation are M-F 8am-7pm and Saturday 8am-5pm.  

Community Health Worker I Job Responsibilities:

  • Completion of CHW Certification after 2,000 hours of work
  • Book appointments in Electronic Health Record, according to providers’ scope of practice.
  • Use Microsoft Office to communicate and report information
  • Perform patient screening surveys; score questionnaires and alert care team
  • Document encounters in electronic health records
  • Participate in health fairs, community and outreach events to promote BACH services
  • Maintain logs as directed by supervisors
  • Participate in health fairs, tabling, collect information
  • Follow-up on no-show appointments
  • Schedule in person interpreters, and transportation, via patient health plans as applicable
  • Be highly dependable and independent-
  • Conduct outreach and enrollment services in available public benefit programs
  • Connect patients to internal resources and/or services to meet their needs
  • Serve as a liaison between the community and health and social services.
  • Interface well with clinic staff, community and care teams.
  • Meet departmental review and documentation standards for work assignment
  • Adhere to the policy and procedures of assigned clinic(s)
  • Build and maintain appropriate relationships on behalf of BACH
  • Support a positive work environment and foster teamwork
  • Support a positive image of BACH inside and outside the organization.
  • Work flexible hours, which includes some evenings and weekends
  • Work at any of BACH location, including clinics, mobiles, schools, homeless encampments, warming centers and patients’ homes, or patient appointments as assigned
  • Attends workshops, training, and meetings as needed, and as requested.
  • Performs other duties as assigned by supervisor.

Community Health Worker I Requirements

  • Two (2) years of experience in a community-based health care service capacity preferred
  • Verifiable High School Diploma or equivalent required
  • Bilingual in Spanish preferred

Work Conditions:

  • Travel to other sites and/or locations required
  • Work outside in varying temperatures may be required
  • Prolonged periods of standing or sitting.
  • Work flexible hours, which includes some evenings and weekends

Physical Requirements:

  • Sitting, walking, and standing associated with office environment
  • Regular bending, stretching and lifting
  • Manual dexterity used to include, but not limited to, standard office equipment, and computer keyboard.
  • Traveling to work sites may include exposure to outside elements
  • Presenting information in front of a group or individual
  • Reading and writing in order to review records for accuracy
