Community Health Worker Summary and Highlights:

Community Health Care Workers (CHW) serve the community as a bridge between the member (community) and the healthcare system (providers) through outreach and education. By providing these services, CHW’s help members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties to become more acculturated with each other.

  • Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance

Community Health Worker Details:

  • Location: Peoria County, IL
  • Payrate: $20/hour
  • Schedule: 8:30am-4:30pm M-F

Community Health Worker Responsibilities:

  • Conduct home visits with members with chronic conditions to reinforce following the care plan
  • Work to reduce cultural and socio-economic barriers between clients and institutions.
  • Identify gaps and opportunities to strengthen systems of care within the member’s community and assists members in utilizing community services
  • Assist with patient medication adherence
  • Teach disease self-management
  • Enter and maintain member records in electronic health record system, compile reports and complete other program documentation

Job Requirements

Community Health Worker Qualifications:

  • At least 1 year of overall related experience conducting home visits
  • 1-2 years’ experience providing clinical services
  • 3-5 years relevant experience
  • Meet all requirements set forth by local state (first aid, CPR, TB screen, etc.)
  • Experience using Excel and Electronic Health Records
  • Experience in community/outpatient setting preferred
  • Community Health Worker Certification or equivalent training and experience preferred
  • High School Diploma or GED
  • Attendance is mandatory for the first 90 days
