The Care Coordinator will be working for a reputable organization with opportunities for career growth. This is a full-time, 1st shift position, working from 7:30 AM to 4:00 PM.

Care Coordinator Compensation
• The pay for this position is $20 to $24 per hour
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Care Coordinator Highlights
• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• The required availability for this position is 1st shift, 7:30 AM - 4:00 PM
• Potential to float to:
- Curran-Fromhold Correctional Facility (CFCF)
- Detention Center
- Riverside Correctional Facility (RCF)
Care Coordinator Responsibilities
• Manage daily processes related to the collection and documentation of patient appointment refusals
• Conduct hourly rounds of the intake area to monitor patient flow in service areas and report barriers to site leaders
• Assist in collecting and managing data related to access to care initiatives as assigned
• Ensure appropriate security measures are followed as outlined by the facility
• Track necessary data to support care coordination initiatives
• Perform other duties as assigned
Care Coordinator Requirements
• Excellent communication, time management, and organizational skills
• High School Diploma or equivalent
• Subject to initial and ongoing security clearance requirements
• Attendance is mandatory for the first 90 days
Care Coordinator Preferred Qualifications
• Prior experience in healthcare coordination, preferably in correctional settings
