Family Nurse Practitioner (FNP) – Float Primary Care / Urgent Care
Location: Schaumburg, Illinois, United States (Float across Chicago metro: Chicago, Evanston, Brookfield, Oak Park)
Pay Rate: $74.87/hour
Hours: Average 30–35 hours/week
Schedule: 3 x 10-hour shifts during weekdays + every other weekend requirement
Family Nurse Practitioner (FNP) Position Overview
- This is a Family Nurse Practitioner (FNP)-required role delivering primary and family-focused care across a multi-site outpatient network
- Providers rotate across 13 clinics based in the Chicago metro area
- Care is delivered in collaboration with a dedicated clinical care team focused on panel management and outcomes improvement
- Role reports to the Practice Manager
FNP Requirement (Critical)
- THIS ROLE REQUIRES ACTIVE FAMILY NURSE PRACTITIONER (FNP) CERTIFICATION
- Candidates without FNP certification will not be considered
- Must have formal training and board certification as a Family Nurse Practitioner
Key Responsibilities
- Provide evidence-based Family and Primary Care across the lifespan (assessment, diagnosis, treatment, and management of acute and chronic conditions)
- Manage assigned patient panels with focus on outcomes, prevention, and chronic disease control
- Conduct routine exams, diagnostic evaluation, and treatment planning
- Deliver patient counseling on wellness, lifestyle modification, and preventive care
- Coordinate care with physicians, nurses, and interdisciplinary clinical teams
- Perform clinic operational duties including opening/closing, inventory, insurance verification, payment collection, and patient follow-up
- Maintain accurate and timely EMR documentation across all assigned sites
- Support behavioral health screening and chronic condition management initiatives
- Ensure consistent patient experience across all 13 clinic locations
Family Nurse Practitioner (FNP) Requirements
- Active FNP certification (required)
- Minimum 1 year recent Primary Care, Family Medicine, or Urgent Care experience (required)
- Active Nurse Practitioner license in Illinois, United States or eligibility to obtain
- Master’s degree in Nursing (Family Nurse Practitioner program)
- National Board Certification required
- Ability to travel between multiple clinic sites (13-location float model)
- Willingness to work every other weekend
- Strong understanding of chronic disease management and quality metrics
- Ability to function independently within a collaborative clinical care team
- Strong communication and documentation skills
- EMR proficiency required
