Family Nurse Practitioner (FNP) Position Overview
Family Nurse Practitioners in this role provide comprehensive, evidence-based primary and family-focused care to a panel of patients within a collaborative clinical care team environment. Providers work closely with physicians, nurses, and support staff to improve outcomes across their patient population. This position reports directly to the practice manager.
The FNP will be responsible for delivering high-quality care through accurate assessment, diagnosis, treatment, and management of both acute and chronic conditions, along with health counseling and disposition planning. The role emphasizes patient-centered care, chronic disease management, and preventive health services.
Family Nurse Practitioner (FNP) Key Responsibilities
- Provide full-scope Family Medicine and Primary Care services, including assessment, diagnosis, treatment, and ongoing management of patient health conditions
- Conduct routine examinations and manage patient appointments to support continuity of care
- Order, perform, and interpret diagnostic tests as clinically appropriate
- Develop and implement individualized treatment and care plans in coordination with the clinical care team
- Deliver patient education focused on wellness, preventive care, lifestyle modification, and chronic disease management
- Support behavioral health integration and health screening initiatives
- Complete administrative duties including clinic opening/closing procedures, inventory management, insurance verification, patient follow-up, and payment collection
- Maintain a clean, compliant, and safe clinical environment
- Collaborate with healthcare professionals to ensure coordinated and comprehensive care delivery
- Encourage patient engagement and active participation in their healthcare outcomes
Family Nurse Practitioner (FNP) Requirements
- Active Family Nurse Practitioner (FNP) certification required
- Minimum of 1 year recent Primary Care, Family Medicine, or Urgent Care experience required
- Active state licensure in South Carolina (or eligibility to obtain)
- Master’s Degree from an accredited Family Nurse Practitioner program
- National Board Certification required
- Strong understanding of chronic disease management and quality metrics in primary care
- Ability to work independently while functioning within a clinical care team model
- Strong communication skills (verbal, written, and electronic)
- Excellent organizational skills with ability to manage multiple priorities
- Proficiency with EMR and healthcare information systems
- Self-directed, adaptable, and able to operate autonomously in a clinical setting
- Willingness to obtain additional multi-state licensure if required

- Ability to manage a patient panel and contribute to quality improvement initiatives
- Comfort with both clinical and administrative responsibilities within a retail-based or outpatient clinic setting
