Job Title: Family Nurse Practitioner
Pay: $51.51 an hr | Bi-weekly
Job Type: Onsite in Edmond, OK
Shift Options:
- Clinic hours, minimum 30/hrs per week. Requires working every other weekend and some major holidays
Family Nurse Practitioner Overview:
- Responsibilities include but are not limited to
Patient-Centered Quality and Safety
• Uses knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional to provide care to patients
• Performs history and physical assessments of patients that are accurate, complete, concise, relevant, and appropriate for age, gender, and clinical symptoms.
• Provides patient counseling; inclusive of pregnancy prevention, STI Prevention/safer sex practices, contraceptive care counseling and medication management
• Counsels and educates patients and families on health maintenance and respond to patient care inquiries
• Evaluates and treats patients' medical needs with safe and high-quality treatment
• Documents all patient care within an electronic health record in accordance with current policies and procedures
• Works as a member of the clinical care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is always provided for all patients
• Provides patient-centered care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness and treatment of disease
• Refers to appropriate specialists and patient care components as appropriate
• Maintain patient confidentiality in accordance with PHI and HIPAA standards
Communication and Interdisciplinary Collaboration
• Collaborates with clinical care team to ensure the delivery of high quality, patient centric, longitudinal care
• Participates in and in some cases leads clinical care team huddles
• Demonstrates core values of CVS Health in all communications and interactions
• Communicates effectively with members of the clinical care team, other health professionals and health-related agencies.
• On call rotation shared with providers in market for after hours patient clinical needs or shift coverage
• Works effectively with clinical care team members
• Promotes mutual respect and trust working within a team model.
• Cultivates and maintains all levels of communication; adjusts communication style to effectively influence quality outcomes and patient needs

• Resolves conflict using appropriate communication and management techniques

• Assist with hiring, development, and evaluation of Primary Care clinical care team as appropriate
• Complete necessary tasks for clinic operations, including but not limited to following up on lab results, and receiving incoming clinical information.

• Continuing education, including what is required to maintain employment at CVS Health/ Primary Care, is the responsibility of the provider
Family Nurse Practitioner Qualifications:
- Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing
• Current National Board Certification, Family Practice
• A minimum of 1 year of recent Primary Care experience as a Nurse Practitioner/Physician Assistant in either a family medicine or primary care practice preferred. If not, successful completion of CVS Minute Clinic primary care training program.
• Working knowledge and understanding of quality measurement in the management of chronic disease conditions
• Self- motivated, prioritizes and solves problems, takes initiative, and advocates for their patients and their practice
• Effective verbal, written, and electronic communication skills
• Outstanding organizational skills and ability to multi-task
• Initiative, problem solving ability, adaptability, and flexibility
• Ability to work remotely in a clinical care team culture
• Ability to work without direct supervision and practice autonomously
• Is proficient with information management and technology
• Willingness to obtain multi-state licensures
• Capacity to collaborate with professional colleagues frequently to ensure quality patient care
