Job Title: Hybrid Field Care Manager

Pay: $40-$45 an hr | Bi-weekly

Job Type:

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs 
• Field Work: Approximately 50–60% in the field, remainder remote documentation
• Must reside in or near Lucas, Fulton, Wood, or Ottawa County, OH
• Must be licensed as a LCSW, LMSW, LMFT, LMHC, or LPC

Shift Options:

• Monday–Friday, 8:00 AM – 5:00 PM EST

Hybrid Field Care Manager Overview:

• Develop, assess, and coordinate complex care management plans for members with primarily physical health needs
• Conduct 5–7 member visits per week (in-home or community-based)
• Educate members and families on care plans, resources, and services
• Complete all documentation within 24 hours of visits
• Coordinate care with providers, community resources, and internal teams
• Manage tasks, emails, and urgent member needs daily
• Meet required turnaround times and performance metrics

Hybrid Field Care Manager Qualifications:

• Education: Master’s degree in a behavioral health or related field (required)
• Licensure (required): LCSW, LMSW, LMFT, LMHC, or LPC
• Experience:
• • 2–4 years of care management, case management, or home health experience
  • Discharge planning and behavioral health experience preferred
• Valid driver’s license and ability to travel locally for member visits
• Strong computer skills (Microsoft Word, Excel, Outlook, Teams)
• Ability to work independently, manage change, and think critically

• Comfortable working both independently and as part of a team
• Flexible, organized, and member-focused
• Able to complete virtual training with camera on and no time off during training

Hybrid Field Care Manager Benefits:
