A-Line Staffing is now hiring a Behavioral Health Care Manager in the Toledo, OH area.
This Behavioral Health Care Manager role will be working for a Major Fortune 500 Healthcare Company and offers strong career growth potential.
Behavioral Health Care Manager Highlights:
- Pay Rate: $40-45/hr, plus mileage reimbursement
- Schedule: Monday–Friday, 8:00 AM – 5:00 PM EST
- Work Model: Hybrid / Field-based (Ohio-based, region-specific)
- Field Work: Approximately 50–60% member-facing in the field
- Remote Work: Documentation completed from home
- Travel: Occasional travel (approximately once per year) to Columbus, OH for team meetings
- Location Requirement: Must reside in or near Lucas, Fulton, Wood, or Ottawa County, OH
Behavioral Health Care Manager Responsibilities:
- Develop, assess, and facilitate complex care management for members with primarily physical healthcare needs
- Create and manage personalized care plans and provide education to members and their families
- Conduct 5–7 scheduled member visits per week
- Review emails, tasks, and voicemails daily beginning at 8:00 AM
- Coordinate care, attend meetings, and collaborate with internal teams as needed
- Complete all required documentation within 24 hours of member visits
- Meet turnaround times for assigned processes and complete all required trainings on time
Behavioral Health Care Manager Qualifications:
- Required: Master’s-level Behavioral Health Professional
- Licensure (must have one):
- LCSW
- LMSW
- LMFT
- LMHC
- LPC
- Experience:
- 2–4 years in care management, case management, or home health
- Discharge planning experience
- Behavioral health experience
- Ability to work independently with minimal supervision
- Strong computer skills (Microsoft tools)
- Ability to travel occasionally for meetings
