Field Case Manager (Bilingual Spanish) Compensation

  • The pay for this position is $34.13 per hour.
  • Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance.
  • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates.

Field Case Manager (Bilingual Spanish) Highlights

  • This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs.
  • The required availability for this position is Monday-Friday working schedule 8am-5pm

Field Case Manager (Bilingual Spanish) Responsibilities

  • Case Management Coordinators will determine appropriate services and supports due to member’s health needs; including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports.
  • Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports.

Field Case Manager (Bilingual Spanish) Requirements

  • One year Case management experience A MUST / Case Management Certificate (Preferred)
  • Long term care experience (Preferred)
  • Microsoft Office including Excel competent
  • Bachelor's degree required-Social work degree or related field.
  • High School Diploma or GED
  • Attendance is mandatory for the first 90 days.
