Field Community Health Worker Compensation

• The pay for this position is $25.00 per hour
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Field Community Health Worker Highlights

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• The required availability for this position is M-F 8-5

Field Community Health Worker Responsibilities

• Promote healthcare equity and assist in addressing Social Determinants of Health for our Maryland Medicaid members
• Help connect our members with community resources (ex. Finding health care providers, arranging transportation to appointments, connecting with a food pantry)
• Outreaching members to see how we can help meet their identified needs, and will be working in conjunction with our care management department

Field Community Health Worker Requirements

• High School Diploma or GED
• Attendance is mandatory for the first 90 days
• Previous healthcare experience
• Considering candidates from the following counties:

• Prince George's Co.
• Anne Arundel Co
• Howard Co
• Baltimore Co.
• Baltimore City
• Frederick Co

Field Community Health Worker Preferred Qualifications

• Maryland State CHW certification
• Bilingual
