Field RN Case Manager (Registered Nurse) Compensation

  • The pay for this position is $35.87 per hour.
  • Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance.
  • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates.

  • The required availability for this position is Monday - Friday 8am - 5pm CST

Field RN Case Manager (Registered Nurse) Responsibilities

  • Nurse Case Manager is responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness.
  • Utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes.

Field RN Case Manager (Registered Nurse) Requirements

  • This is a telework role that will require 50-75% travel within the Bureau County, IL
  • Licensed Registered Nurse in the State of Illinois
  • RN, LCSW, or LCPC with current unrestricted state licensure in IL. REQUIRED

Minimum 3-5 years clinical practical experience preferred
Minimum 2-3 years Care Management, discharge planning and/or home health care coordination experience preferred
Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
