RN Case Manager | Details and Compensation:
- Location: Lakewood, CO 80228 – Field work required
- Payrate: $56.14/hr
- Required Availability: Mon-Fri 8a-5pm
RN Case Manager | Summary and Highlights:
The Registered Nurse (RN) Case Manager will perform comprehensive assessments of high-risk members and collaborate with primary care providers to ensure the implementation of individualized care plans. You will play a crucial role in coordinating care transitions, engaging specialty resources, and advocating for patients to achieve optimal health outcomes.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
RN Case Manager | Responsibilities:
- Perform comprehensive assessments of high-risk members.
- Collaborate with primary care providers to develop and implement individualized care plans.
- Address, identify, and continuously reassess cost-efficient, appropriate levels of care.
- Put non-medical support in place to ensure compliance with treatment plans (e.g., housing, transportation).
- Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.
- Engage specialty resources as needed.
- Maintain detailed records of clinical, functional, and fiscal outcomes.
- Identify opportunities for health promotion and illness prevention.
- Prevent adverse patient occurrences and intervene quickly when necessary.
- Act as a patient advocate, protecting privacy and confidentiality.
- Onsite work required
RN Case Manager | Requirements:
- Education: Associate's or Bachelor's Degree in Nursing
- Current, unrestricted Registered Nurse (RN) license in the state of Colorado.
- Ability to analyze complex medical information and make rational decisions.
- Case Management certification is preferred.
- Attendance: Commitment to mandatory attendance during the first 90 days.
