RN Case Manager | Details and Compensation:

  • Location: Lakewood, CO 80228 – Field work required
  • Payrate: $56.14/hr
  • Required Availability: Mon-Fri 8a-5pm

RN Case Manager | Summary and Highlights:

The Registered Nurse (RN) Case Manager will perform comprehensive assessments of high-risk members and collaborate with primary care providers to ensure the implementation of individualized care plans. You will play a crucial role in coordinating care transitions, engaging specialty resources, and advocating for patients to achieve optimal health outcomes.

  • This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
  • Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
  • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

RN Case Manager | Responsibilities:

  • Perform comprehensive assessments of high-risk members.
  • Collaborate with primary care providers to develop and implement individualized care plans.
  • Address, identify, and continuously reassess cost-efficient, appropriate levels of care.
  • Put non-medical support in place to ensure compliance with treatment plans (e.g., housing, transportation).
  • Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.
  • Engage specialty resources as needed.
  • Maintain detailed records of clinical, functional, and fiscal outcomes.
  • Identify opportunities for health promotion and illness prevention.
  • Prevent adverse patient occurrences and intervene quickly when necessary.
  • Act as a patient advocate, protecting privacy and confidentiality.
  • Onsite work required

RN Case Manager | Requirements:

  • Education: Associate's or Bachelor's Degree in Nursing
  • Current, unrestricted Registered Nurse (RN) license in the state of Colorado.
  • Ability to analyze complex medical information and make rational decisions.
  • Case Management certification is preferred.
  • Attendance: Commitment to mandatory attendance during the first 90 days.
