Field RN Case Manager Compensation

• Pay: $56.14 per hour

• Benefits available after 90 days of employment

• 401(k) with company match available after 1 year of service

Field RN Case Manager Highlights

• Contract assignment with the potential for permanent hire based on performance, attendance, and business needs

• Location: Onsite in Nashville, TN

• Required schedule: Monday – Friday, 8 AM – 5 PM

Field RN Case Manager Responsibilities

• Perform comprehensive assessments of high-risk members

• Collaborate with primary care providers to develop individualized care plans

• Address cost-efficient and appropriate levels of care

• Assist with non-medical support (housing, transportation, etc.)

• Coordinate care transitions and collaborate with social workers, discharge planners, and other professionals

• Engage specialty resources as needed for optimal patient outcomes

• Maintain detailed records of clinical, functional, and fiscal outcomes

• Identify opportunities for health promotion and illness prevention

• Advocate for patients while maintaining privacy and confidentiality

Field RN Case Manager Requirements

• Associate’s or Bachelor’s degree in Nursing or a related field

• Active and unrestricted Registered Nurse (RN) license

• Ability to analyze complex medical information and make informed decisions

• Case Management certification preferred

• Mandatory attendance for the first 90 days
