Field RN Case Manager Compensation

• The pay for this position is $48.69 per hour
• Mileage is reimbursed
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Field RN Case Manager Highlights

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• The required availability for this position is Monday – Friday, 8:00 AM – 4:30 PM
• One weekend on-call per month is required
• 4–6 patient visits per day
• Travel between patients is required

Field RN Case Manager Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans
• Provide health maintenance, disease prevention, and case management
• Coordinate communication between healthcare professionals, ensuring seamless care transitions
• Oversee medication management, including ordering, monitoring, and disposal per physician orders
• Ensure compliance with medication administration policies and update as necessary
• Document patient health status in nurse’s notes, ensuring accuracy and compliance with regulations
• Supervise and direct licensed practical/vocational nurses (LPN/LVN) as needed
• Provide training to unlicensed staff on person-specific health and safety needs
• Schedule and coordinate health-related appointments, ensuring appropriate follow-up care
• Maintain communication with inpatient nursing and discharge coordinators for hospitalized patients
• Ensure medical supplies and equipment are maintained within compliance guidelines

Field RN Case Manager Requirements

• Current unrestricted Tennessee RN License or Compact State License
• Home health experience – Required
• Proficiency with Oasis Assessment form – Required
• HCHB (Home Care Home Base) experience – Highly Preferred (If no HCHB experience, candidates must be technologically savvy)
• High School Diploma or GED
• Attendance is mandatory for the first 90 days
