Field RN Case manager Compensation

  • The pay for this position is 45.00-48.00 per hour Weekly paid
  • Benefits are available to full-time employees after 90 days of employment

Field RN Case manager Highlights

  • The required availability for this position is Monday thru Friday 8am - 5pm  with on call rotation

Field RN Case manager Responsibilities

  • In addition to assessing the health of the people we serve and implementing their care plans the Registered Nurse will ensure health maintenance and disease prevention and/or provide case management.
  • Supervision of care plans and staff members may also be involved in daily work assignments. Duties of this healthcare role will include but are not limited to: Observing assessing planning implementing and evaluating nursing support for well chronic or acutely ill individuals in a community or small facility setting.
  • Working closely with other health professionals/providers in planning implementing and evaluating programs and services to address the health needs of persons served. Coordinate communications and actions of various disciplines as it relates to health and safety supports. Ensuring the ordering reviewing monitoring discontinuation and if necessary the disposal of medications as prescribed by the physician.

Field RN Case manager Requirements

  • Active RN License in the state or territory you are working in: Multiple state license 4--6 visits daily
  • Must have home health experience HCHB-desired not required
  • High recommend Must Technologically Savvy-only
  • Travel between patience Mileage is reimbursed at .41 cents per mile
  • Valid state license required specific to the state in which the associate works
  • Diploma from an accredited school/college of nursing OR required professional licensure at time
