Field Registered Nurse (RN) – LTSS / UAS Assessor Compensation

• The pay for this position is $35.49 – $63.79 per hour.
• Benefits are available to full-time employees after 90 days of employment.
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates.

Field Registered Nurse (RN) – LTSS / UAS Assessor Highlights

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• Position is field-based, conducting UAS Assessments and Reassessments
• Candidates must reside in Steuben County, NY and will cover Steuben, Chemung, Schuyler, and Yates counties
• Annual bonus incentive opportunity of 8%
• The required availability for this position is Monday–Friday, full-time field schedule, completing approximately 2–4 UAS assessments per day.

Field Registered Nurse (RN) – LTSS / UAS Assessor Responsibilities

• Conduct in-home and site-based UAS assessments for long-term care members
• Assess, coordinate, and manage medical and supportive services for complex/high-acuity populations
• Develop, implement, and update individualized care/service plans
• Coordinate care with members, families, caregivers, providers, and community resources
• Monitor member condition, progress toward goals, and need for changes in level of care
• Educate members and caregivers on benefits, services, disease processes, and care options
• Document all care management activities in compliance with state and federal regulations
• Act as a liaison and advocate between members, providers, and service agencies
• Support onboarding and precepting of new clinical staff as needed
• Perform other duties as assigned

Field Registered Nurse (RN) – LTSS / UAS Assessor Requirements

• Active NY Registered Nurse (RN) license
• UAS assessment experience required
• Minimum of 4 years of relevant RN experience
• Ability and willingness to work fully in the field Monday–Friday
• Ability to conduct home and/or site visits
• Valid driver’s license (personal transportation preferred; public transportation considered)
• Strong assessment, care coordination, and communication skills
• High School Diploma or GED
• Attendance is mandatory for the first 90 days.

Field Registered Nurse (RN) – LTSS / UAS Assessor Preferred Qualifications

• Bachelor’s degree in Nursing (BSN)
• Experience in care management, preferably within a Managed Care Organization (MCO)
• Experience working with complex or high-acuity populations
• Knowledge of community-based and long-term care resources
• Leadership or precepting experience
