Job Description
- Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals.
- Complete comprehensive evaluations using appropriate assessment tools and procedures.
- Establish measurable goals and develop treatment plans to enhance patient independence and function.
- Regularly reassess treatment plans and modify as needed based on patient progress.
- Communicate patient needs and progress to treatment teams and family members.
- Maintain and submit evaluation, treatment, and billing documentation according to required standards.
- Supervise Physical Therapist Assistants and Aides in accordance with state practice acts.
- Shift: Monday - Friday 8am - 5pm EST, Onsite
- County Covered: Boyd, Greenup, Lawrence, Carter, Elliot
- KY Physical Therapist Licensure, Compact Licensure Accepted
- 1 year home health physical therapy experience, previous oasis experience
- Benefits are available to full-time employees after 90 days of employment.
