Job Description
Job Title: Home Care Physical Therapist
Location: Lexington KY 40505
Coverage: 50 miles within branch
Hours: M-F 8am-5pm (May have some on call when designated on call person is not unable)
Pay: $63.00 - $65.00 per hour – not per visit
Scheduled pay: Bi-Weekly
Mileage Reimbursement: yes
Benefits: Health, Dental, Vision, Life Insurance, and Short term disability after 90 days

Home Care Physical Therapist Responsibilities
- 30 units per week

Conduct comprehensive patient evaluations in home settings to assess mobility, strength, balance, functional abilities, and overall physical condition.

Develop individualized treatment plans based on physician orders, patient goals, and functional assessments.

Provide hands-on therapy, including exercises, manual techniques, gait training, balance and coordination activities, and range-of-motion exercises.

Educate patients and caregivers on home exercise programs, safety techniques, and strategies to prevent falls or injury.

Monitor and document patient progress, updating treatment plans as needed to achieve optimal outcomes.

Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate care and ensure continuity of treatment.

Assist patients with adaptive equipment and recommend home modifications to improve safety and independence.

Provide patient and family education on disease processes, recovery expectations, and self-management techniques.

Maintain compliance with state and federal regulations, company policies, and professional standards, including HIPAA.

Participate in interdisciplinary team meetings to discuss patient progress, barriers to care, and discharge planning.

Communicate effectively with patients, families, and caregivers to address concerns, motivate adherence, and ensure a positive care experience.

Travel to patient homes safely, maintaining professional standards in varying home environments.
Job Requirements
Home Care Physical Therapist Requirements:
- Bachelor of Science in Physical Therapy from an APTA approved program - Required.
- Current State License as a Physical Therapist - Required.
- Proof of current BLS certification - Required.
- Drivers license- required
- 1-2 years experience in an acute care or rehabilitation setting - Required.
