A-Line Staffing is now hiring LTSS Service Care Manager in the Pearsall, TX area. The LTSS Service Care Manager would be working for a Major Health Plan and has career growth potential.

LTSS Service Care Manager Highlights:

• Schedule: 8:00am – 5:00pm, Monday through Friday; Overtime may be required
• Pay Range: $23-27 Hourly (based on experience)
• 75% travel required – mileage reimbursement provided
• Company laptop and work cell phone provided

LTSS Service Care Manager Responsibilities:

• Travel 3 days/week within the MRSA West Area to conduct home visits; work remotely 2 days/week for documentation and follow-ups
• Perform assessments of aged, blind, or disabled members to determine need for personal attendant services
• Complete and submit H2060 and related forms within 3 business days of assessment
• Develop and recommend care plans for high-risk members
• Act as a liaison and advocate between members, families, physicians, and community agencies
• Educate members and caregivers on preventive care, treatment options, community resources, and benefits
• Coordinate authorizations for services including meals, housing, ADLs, and transportation
• Maintain accurate electronic documentation per clinical and compliance standards
• Provide feedback to leadership for improved service delivery
• Ensure quality standards of 92%+ are consistently met

LTSS Service Care Manager Qualifications:

• Required: Bachelor’s degree and 2–4 years of related experience OR LVN and 2–4 years of equivalent experience
• Preferred: Background in Social Work, LVN, or other medical-related fields
• 5+ years experience working with the aged, blind, or disabled population
• Strong proficiency in Microsoft Office (Outlook, Word, Excel)
• Bilingual (English/Spanish) strongly preferred
• Excellent interpersonal, organizational, and documentation skills
