A-Line Staffing is now hiring Home Care RN in Union / Somerset County. The Home Care RN would be working for a Major Healthcare System and has career growth potential.

Home Care RN Highlights:

• Schedule: Monday – Friday, 8:00 AM – 4:30 PM
• Pay Range: $60-$70 hourly, plus mileage reimbursement

Home Care RN Responsibilities:

• Assess home health/hospice patients in stable and emergent situations to determine physical, psychosocial, and environmental needs.
• Develop, implement, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Incorporate physical, psychological, social, spiritual, and cultural factors into the POC.
• Maintain up-to-date patient records with accurate documentation.
• Provide care using infection control measures (OSHA guidelines).
• Educate patients and families on care, diet, medications, and home safety.
• Supervise and provide clinical direction to HHA/STNA/LVN staff.
• Assess and manage pain using appropriate pain management techniques.
• Arrange referrals to external agencies when necessary.
• Monitor compliance with third-party payer requirements.
• Provide on-call services as assigned.
• Communicate effectively with patients, families, physicians, and healthcare professionals.
• Attend interdisciplinary team meetings and other required meetings.
• Perform other duties as assigned.

Home Care RN Required Qualifications:

• Homecare Homebase Software experience
• OASIS experience
• Current Registered Nurse (RN) license
• Proof of CPR certification
• Current driver’s license & reliable transportation with insurance
• 1-2 years of clinical RN experience
• 2+ years of home health experience

Preferred Qualifications:

• CHPN certification
• Bachelor of Science in Nursing (BSN)

Minimum Education:

• Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma program
