HOME HEALTH LPN | DETAILS AND COMPENSATION:
- Location: Camden AL 36726 – 100% On-site in home health settings
- Payrate: $37.30/hr + mileage, $0.48
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM

- Occasional weekends may be required due to weather-related or make-up appointments
HOME HEALTH LPN | SUMMARY AND HIGHLIGHTS:
The Home Health LPN will provide high-quality patient care in accordance with individualized plans of care. This role is ideal for an LPN who is comfortable working independently, values patient education, and enjoys consistent weekday hours with occasional flexibility.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
HOME HEALTH LPN | RESPONSIBILITIES:
- Provide nursing care following established plans of care while maintaining patient dignity and independence
- Monitor vital signs and report abnormal findings promptly
- Administer medications, injections, feedings, oxygen, and ostomy care as ordered
- Perform routine clinical tasks as directed by interdisciplinary care teams (PT, OT, Speech Therapy)
- Educate patients and caregivers on disease processes and catheter care as needed
- Ensure infection control policies are consistently followed
- Accurately document nursing care, patient progress, and outcomes
- Order and maintain appropriate levels of medical supplies and medications
- Monitor incidents, complete required documentation, and notify appropriate personnel
- Ensure medical equipment is properly maintained and report issues for repair
HOME HEALTH LPN | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable) required
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active Alabama LPN license
- Ability to work onsite in home health settings
- Strong documentation and communication skills
- Commitment to patient safety and quality care
- Preferred: Wound care (including wound vacs), Experience with injections, Knowledge of heart failure and diabetes management, Patient education experience related to disease processes, Experience with Homecare Homebase (HCHB)
