Home Health Nurse Compensation

  • The pay for this position is 48.00 per hour
  • Benefits are available to full-time employees after 90 days of employment
  • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Home Health Nurse Highlights

  • This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
  • The required availability for this position is Monday to Friday, until shift is complete. On-call every 2-3 weeks.

Home Health Nurse Responsibilities

  • Administer skilled nursing care for patients in their home environment.
  • Assess home health and hospice patients in both stable and emergent situations to identify physical, psychosocial, and environmental needs.
  • Develop, document, and evaluate individualized Plans of Care (POC) to ensure quality and continuity of care.
  • Obtain data regarding physical, psychological, social, spiritual, and cultural factors affecting patient health, integrating these into the Plan of Care.
  • Ensure continuity of care with up-to-date patient records, accurately reflecting visits, contacts, goals, and any changes in the patient's condition.
  • Adhere to infection control standards, protecting both the patient and staff.
  • Provide education to patients and families about care, medications, home safety, and self-care practices using appropriate teaching methods.
  • Supervise and provide clinical direction to Home Health Aides (HHA), STNA, or LVN to maintain service quality.
  • Assess and manage pain, utilizing appropriate techniques, and educate patients and families on pain management.
  • Make referrals to appropriate agencies when needed.
  • Monitor cases to ensure compliance with third-party payer requirements.
  • Perform on-call responsibilities and provide services to patients and families as needed.
  • Communicate effectively with patients, families, interdisciplinary teams, and healthcare professionals.
  • Attend interdisciplinary team meetings and other meetings as necessary.
  • Perform other duties as assigned.

Home Health Nurse Requirements

  • Active Registered Nurse (RN) license in the state of Indiana.
  • Minimum of 1-2 years of experience as a Registered Nurse in a clinical care setting.
  • Minimum of 2 years of home health experience.
  • Experience with Homecare Homebase and OASIS required.
  • Proof of current CPR certification. High school diploma or GED required
