Home Health Physical Therapist (PT) Schedule

Monday–Friday, 8:00am–5:00pm

Saturday coverage once per month for weekend admissions
Home Health Physical Therapist (PT) Position Summary
Provides home health physical therapy evaluations and treatments based on physician orders. Develops individualized care plans to maximize patient independence and functional outcomes. Collaborates with interdisciplinary teams and communicates patient progress, needs, and discharge recommendations.
Home Health Physical Therapist (PT) Key Responsibilities

Perform comprehensive PT evaluations using appropriate assessment tools

Review medical history, diagnoses, precautions, and contraindications

Establish measurable goals and treatment plans

Reassess progress and adjust interventions as needed

Educate patients and families on therapy programs and equipment use

Coordinate care with physicians and clinical team members

Recommend adaptive equipment to improve function

Maintain timely and compliant clinical documentation and billing records

Supervise PTAs and therapy aides per state practice act

Participate in staff meetings, training, and continuing education
Home Health Physical Therapist (PT) Requirements

Active Physical Therapist (PT) license
-
Ability to evaluate clinical information and develop effective treatment plans

Strong documentation and communication skills
Home Health Physical Therapist (PT) Preferred Qualifications

OASIS assessment experience
-
Adult therapy background
Compliance
Must adhere to HIPAA, company compliance standards, professional practice regulations, and all state/federal guidelines.
