Home Health Physical Therapist | Details and Compensation:

• Location: Albemarle, NC 28001 – 100% on-site + fieldwork
• Payrate: $57.30/hr
• Required Availability: Mon-Fri 8a-5p

Home Health Physical Therapist | Summary and Highlights:

The Home Health Physical Therapist role involves performing physical therapy evaluations and treatments based on physician referrals. The ideal candidate will be passionate about patient care, have strong communication skills, and be comfortable working in a collaborative healthcare setting.

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Home Health Physical Therapist | Responsibilities:

• Evaluate patients using appropriate tools and procedures
• Review medical history and records for contraindications and precautions
• Develop and implement individualized treatment plans with measurable goals
• Regularly reassess patient progress and modify plans as needed
• Collaborate with physicians, treatment teams, patients, and families
• Recommend and assist in obtaining adaptive equipment for improved function
• Maintain thorough and timely documentation for evaluations, treatments, and billing
• Ensure compliance with HIPAA, company policies, and professional standards
• Supervise PT Assistants and Aides in accordance with state regulations
• Participate in staff development and continuing education
• Fieldwork required

Home Health Physical Therapist | Requirements:

• Education: High School Diploma or GED (Verifiable)
• Attendance: Commitment to mandatory attendance during the first 90 days.
• Current Physical Therapist license (state-specific)
• CPR Certification
• Valid Driver’s License & Car Insurance
• COVID-19 vaccination status disclosed
• Fieldwork required
