Home Health Registered Nurse – Compensation & Schedule

• Pay Rate: $48.69 per hour

• Schedule: Monday–Friday, 8:00 AM – 5:00 PM

• No weekends and no on-call requirements

• Weekday holidays may be required

• Full-time, 40+ hours per week

Home Health Registered Nurse – Position Overview

The Home Health Registered Nurse provides comprehensive nursing care in a community-based setting, focusing on patient assessment, care coordination, disease prevention, and case management. The RN collaborates closely with physicians and interdisciplinary teams to support individuals with acute, chronic, or preventive health needs.

This is a contract assignment with potential for permanent hire based upon attendance, performance, and business needs.

Home Health Registered Nurse – Responsibilities

• Assess, plan, implement, and evaluate individualized nursing care plans

• Provide case management and promote health maintenance and disease prevention

• Coordinate care with physicians, providers, and interdisciplinary teams

• Manage medication ordering, monitoring, documentation, and proper disposal

• Communicate changes in patient condition to physicians and care teams

• Complete required nursing documentation (daily, monthly, quarterly, and annual)

• Transcribe physician orders and medically related reports

• Coordinate care during hospital admissions and discharges

• Supervise Licensed Practical Nurses as required

• Provide new hire training and ongoing health and safety education

• Ensure infection control standards and proper equipment maintenance

• Schedule and follow up on health-related appointments

Home Health Registered Nurse – Requirements

• Active Registered Nurse (RN) license

• Licensed in NC or willing to work under a Compact RN license

• Ability to work independently in a field-based environment

• Strong documentation, communication, and care coordination skills

• Mandatory attendance for the first 90 days

Preferred Qualifications:

• Home health experience

• Homecare Homebase (HCHB) experience

• Wound care and wound vac experience

• IV therapy and port access experience
