Hourly Pay: $48.69/hr.
Location: Cranston, RI 02907 (Providence area)
Schedule: Mon-Fri 8:00am-5:00pm + weekend on-call
Mileage reimbursement: $0.43/mile
Overview
We are hiring a Home Health Registered Nurse (RN) for supporting individuals in a community/small facility setting. In this role, you’ll assess patient needs, implement care plans, coordinate across disciplines, and ensure timely, accurate documentation.
Key Responsibilities
- Observe, assess, plan, implement, & evaluate support for individuals (well, chronic, or acutely ill)
- Coordinate care with providers & interdisciplinary teams to support health & safety needs
- Provide case management and health maintenance/disease prevention support as needed
- Support medication management processes (ordering/review/monitoring/discontinuation)
- Communicate changes in condition and provide progress updates to physicians as needed
- Help ensure infection prevention practices and maintenance of equipment/supplies
- Coordinate health-related appointments and follow through on findings/action plans
Job Requirements
Required Skills & Qualifications
- Active, unencumbered Registered Nurse (RN) license
- Ability to attend weekly case conferences:
- Tuesdays at 8:30 AM (approx. 1.5 hours)
- Wednesdays at 8:45 AM
- Timely documentation within 24 hours
- Comfortable working on-site in the Providence/Cranston, RI area
- Experience with Homecare Homebase (HCHB) and/or PointClickCare
Preferred Qualifications
- Comfort using Microsoft Teams for communication
