Job Title: Home Health RN
Location: Lancaster PA 17601
Coverage Areas: Harrisburg, New Cumberland, Carlisle , Etters, Mechanicsburg, Wormleysburg, Middletown, Hummelstown, Enola, Dillsburg
Hours: 32 hours per week
Days Per Week: 5 days per week- Monday - Friday
Pay: $48.00 - $50.00 per hour – not per visit
Scheduled pay: Weekly
Job Type: Full Time

Home Health Registered Nurse (RN) Responsibilities
- Assess and evaluate the patient’s physical, psychological, and mental needs
- Educate the patient and their family on end-of-life care
- Develop and implement a plan of care that addresses the patient’s needs
- Monitor the patient’s progress and adjust the plan of care as needed
- Maintain patient records and provide written reports on the patient’s condition
- Communicate effectively with other members of the healthcare team
- Participate in team meetings and provide input on patient care

Home Health RN (Registered Nurse) Requirements
- Associates Degree of Nursing Required
- Active RN PA State License
- Valid driver’s license
- Minimum 2 years Home Health experience
- Oasis experience highly preferred
- Home Care Home Base experience required
