Home Health Registered Nurse (RN)
Pueblo, CO (Onsite – Pueblo proper and west coverage area)
$50–$55 per hour | Paid Bi-Weekly
Full-Time | Contract-to-Hire
Position Overview
This position is a contract assignment with the potential for permanent hire based on attendance, performance, and business needs.
The Home Health Registered Nurse (RN) administers skilled nursing care to patients of all ages in their place of residence. The RN coordinates care with the interdisciplinary team, patients/families, and referring agencies, and assumes responsibility for the overall coordination of care.
Schedule
- Monday–Friday | 8:00 AM – 5:00 PM
- Participation in on-call rotation required (includes weekends)
Essential Responsibilities
- Assess home health and/or hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
- Develop, implement, document, and evaluate individualized Plans of Care (POC).
- Incorporate physical, psychological, social, spiritual, and cultural factors into patient care planning.
- Maintain accurate, up-to-date clinical documentation for all visits, contacts, and care updates.
- Follow infection control measures (OSHA compliance).
- Educate patients and families on care plans, medications, diet, home safety, and self-care practices.
- Supervise and provide clinical direction to HHA/STNA/LVN staff.
- Assess and manage pain using appropriate techniques and education.
- Arrange referrals to appropriate agencies as needed.
- Ensure compliance with third-party payer requirements.
- Perform on-call responsibilities as assigned.
- Serve as liaison between patients, families, physicians, administration, and healthcare team members.
- Attend interdisciplinary and team meetings.
- Perform other related duties as assigned.
Required Skills & Competencies
- Strong assessment and clinical decision-making skills
- Effective communication across patients, families, and healthcare teams
- Knowledge of medication administration and IV/venipuncture care
- Competency in:

- Wound staging, measurement, and treatment
- Inhalation therapy
- Phlebotomy
- Bowel and bladder training programs
- Indwelling catheter and drain care
- Safe use of durable medical equipment
- Restraint use in home settings under physician orders
- Ability to prioritize and function in high-stress environments
- Maintain confidentiality
- Ability to work independently and collaboratively
- Flexible schedule with local travel required
Qualifications (Required)
- Current State RN License
- 1–2 years RN experience in a clinical care setting
- 2+ years of home health experience
- OASIS experience
- Homecare Homebase software experience
- Current CPR certification
- Valid driver’s license
- Reliable transportation with insurance
Preferred
- CHPN Certification
- Bachelor of Science in Nursing (BSN)
Education
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program (Required)
- Bachelor of Science in Nursing (Preferred)
