$55–$60 per hour | Paid Bi-Weekly
Full-Time | Contract-to-Hire
Position Overview
This position is a contract assignment with the potential for permanent hire based on attendance, performance, and business needs.
The Home Health Registered Nurse (RN) administers skilled nursing care to patients of all ages in their place of residence. The RN coordinates care with the interdisciplinary team, patients/families, and referring agencies, and assumes responsibility for the overall coordination of care.
We are seeking a compassionate and skilled RN with strong clinical judgment, excellent communication skills, and the ability to work independently in a home-based setting.
Schedule
Monday–Friday | 8:00 AM – 5:00 PM
No call
No weekends
Essential Responsibilities
- Assess home health and/or hospice patients in stable and emergent situations to identify physical, psychosocial, spiritual, cultural, and environmental needs.
- Develop, implement, document, and evaluate individualized Plans of Care (POC).
- Maintain accurate, timely, and comprehensive clinical documentation.
- Ensure continuity and quality of care through ongoing patient monitoring and case management.
- Provide patient care using proper infection control measures (OSHA compliant).
- Educate patients and families regarding disease management, medications, diet, home safety, and self-care practices.
- Supervise and provide clinical direction to HHA/STNA/LVN staff.
- Assess and manage pain using appropriate interventions and patient education.
- Coordinate referrals to appropriate community agencies and resources as needed.
- Monitor assigned cases for compliance with third-party payer requirements.
- Serve as a liaison between patients, families, physicians, and healthcare team members.
- Attend interdisciplinary team meetings.
- Perform additional duties as assigned.
Clinical Competencies
- Medication administration and management
- Venipuncture and IV therapy
- Surgical wound and decubitus ulcer staging and treatment
- Inhalation therapy
- Phlebotomy for outpatient lab testing
- Bowel and bladder training programs
- Care and maintenance of indwelling catheters and drains
- Safe use of durable medical equipment in the home
- Safe use of physician-ordered restraints in the home setting
Qualifications (Required)
- Current State RN License
- 1–2 years RN experience in a clinical care setting
- 2+ years of home health experience
- OASIS experience
- Homecare Homebase software experience
- Current CPR certification
- Valid driver’s license
- Reliable transportation with active insurance
Preferred
- CHPN Certification
- Bachelor of Science in Nursing (BSN)
Education
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program (Required)
- Bachelor of Science in Nursing (Preferred)
