HOME HEALTH RN | DETAILS AND COMPENSATION:
- Location: Clayton County – 100% On-site, home health setting
- Payrate: $60.68/hr + mileage
- Required Availability: Full-Time | Monday–Friday
- 40 hrs (can do 5 8-hr shifts or 4 10-hour)
HOME HEALTH RN | SUMMARY AND HIGHLIGHTS:
The Home Health RN will provide comprehensive case management and clinical oversight for individuals in community or small facility settings, focusing on health maintenance and disease prevention. This role is responsible for implementing care plans, supervising nursing staff, and managing medication protocols to ensure patient safety and regulatory compliance.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs

HOME HEALTH RN | RESPONSIBILITIES:
- Patient Assessment: Assess health status, implement care plans, and provide case management for chronic or acutely ill individuals.
- Clinical Oversight: Supervise LPN/LVN staff and ensure unlicensed staff are trained on person-specific health maintenance and safety.
- Care Coordination: Work closely with multidisciplinary teams, physicians, and discharge coordinators to ensure seamless transitions of care.
- Medication Management: Oversee the ordering, monitoring, and disposal of medications; ensure all administration policies are followed and reviewed annually.
- Documentation: Maintain accurate nurse’s notes and complete daily, monthly, and annual documentation in compliance with regulatory standards.
- Training: Provide new employee training and annual recertifications on health and safety-related topics.
HOME HEALTH RN | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- License: Active and unencumbered Registered Nurse (RN) license.
- Certification: Current CPR-BLS certification.
- Systems Proficiency: Direct experience with HCHB (Homecare Homebase) and OASIS documentation is required.
- Compliance: Ability to travel within the assigned county and work onsite in Fayetteville.
- Experience with the following clinical competencies is highly desired: IV Therapy & Lab Draws, Wound Care / Wound Vac, Tube Feeds, Catheter Changes, Injections
