Compensation

• The pay for this position is $55 per hour.
• Benefits are available to full-time employees after 90 days of employment.
• A 401(k) with a company match is available for full-time employees after 1 year of service on eligibility dates.

Responsibilities

• Administer skilled nursing care for patients of all ages in their home environment.
• Coordinate care with interdisciplinary teams, patients/families, and referring agencies.
• Perform comprehensive assessments of physical, psychosocial, and environmental needs of patients.
• Develop and evaluate the Plan of Care (POC) ensuring quality and continuity of care.
• Educate patients and their families about care plans, medications, safety measures, and more.
• Provide on-call services as needed and maintain accurate patient documentation.
• Supervise and guide the care provided by Home Health Aides or other support staff.
• Assess and manage patient pain and implement effective pain management techniques.

Requirements

• High School Diploma or GED required.
• Current state license as a Registered Nurse required.
• 1-2 years of experience as a Registered Nurse in a clinical care setting required.
• Home Health experience, including OASIS (Outcome and Assessment Information Set), is required.
• Must have reliable transportation and insurance.
• Current CPR certification is required.
• IV/infusion, trach, wound care including wound vac experience required.
• Experience with Homecare Homebase Software and OASIS is required.

Preferred Qualifications

• Bachelors of Science in Nursing preferred.
• CHPN (Certified Hospice and Palliative Nurse) certification preferred.

• Shift: Full-time, 40 hours per week.
• Guaranteed Hours: 32 hours per week.
• On-Call Requirements: Every 4-5 weekends.
